What are my payment options?
We offer self-pay options and also accept many insurances. We accept cash, check, Visa, and MasterCard. For a list of insurances we currently accept please scroll to the bottom of the screen or call our office.
What are my insurance benefits?
Please familiarize yourself with your mental health benefits by contacting your insurance company to ensure preauthorization is not needed prior to initial appointment and to ensure services/provider are covered.
What is your cancellation policy?
We require 24 hrs notice prior to any cancelled/rescheduled appointments otherwise, you will be charged a $70 cancellation fee. Please arrive to your appointment on-time. If you arrive 15 minutes after your scheduled appointment time, you will be charged the $70 cancellation fee and your appointment rescheduled.
Where can I find patient forms?
Please complete these forms IN THEIR ENTIRETY prior to your appointment. Failure to do so may result in your appointment being rescheduled and a fee charged.
When do I pay for service?
Payment is expected at time of service.
When should I arrive for my appointment?
Arrive 15 minutes early. If you did not complete the new patient paperwork please arrive 30 minutes early. Please note that if you do not have the paperwork completed before the appointment and do not arrive early, your appointment may be rescheduled and charge a fee.
What do I need to bring with me?
Please bring Photo ID, Insurance Card, List of Medications, Doctor’s Name & Phone Number.
If you have had treatment in the past & have documents from this, bring those as well.
Do I need to do anything?
Create list of important information or questions for provider.
There are several other insurances we may accept on a provider-by-provider basis, therefore it is best to either call your insurance company and check in-network benefits with the provider you are seeking or call our office directly to see if we have a provider in your network.
We do NOT accept Medicaid at this time.
